Home work and Documents

Our initial interview is free.  But it is not just a meet and greet interaction.  We perform a detailed review of the application profile during the meeting in order to identify issues and create estimates for further work.  Therefore, the more information that you can bring to us at the initial meeting, the better we can estimate the nature of the issues involved and the more accurate our initial fee estimates will be.
 
The links below will download .pdf files a checklist of documents and an information form which we would encourage you to print, review, and complete to the fullest extent possible before you come in.
 
DO NOT POSTPONE OR DELAY AN INTAKE MEETING SIMPLY BECAUSE YOU DON'T HAVE, OR KNOW, ALL THIS STUFF.
 
Medicaid is time sensitive.  DHS takes a snapshot of the finances of an applicant as of 12:01 AM on the first day of any given month.  If the applicant is eligible as of that date, they are eligible all month.  If the applicant is NOT eligible as of that moment, then the applicant misses Medicaid for the entire month.
